Care Navigation Coordinator REQ#4965

The top candidate for this position is responsible to support the Care Navigation Program. This position will interact with clinical care navigator team members, patients, Providers and other stakeholder.

Successful candidate will need to perform the essential functions of the job:

· Engages with identified beneficiaries to drive service/program awareness and facilitate enrollment into our clinical  services and programs

· Accurately documents beneficiary interaction and appropriately follows all processes and procedures related to beneficiary engagement

· Active surveillance of remote patient monitoring alerts for triage and escalation

· Responds to incoming Provider and patient calls

· Ensures effective communication and collaboration with multidisciplinary patient care teams

· Interacts in a collegial and collaborative fashion with outside clinical saff such as specialists, social workers, and other clinical and nonclinical support staff

In order to be qualified for this role, the following is required:

· 2+ years of customer service experience analyzing and solving customer problems OR 1 year experience in an office setting environment using the telephone and computer as the primary instrument to perform job duties

· High school diploma or GED

· Knowledge of medical terminology, anatomy, physiology, and pathophysiology preferred
