Scope of Responsibilities

Responsible for oversight of the Care Management Team.  Develop and maintain data collection tools for auditing patient census and patient details within Care Management, tracking outcomes and hospitalization and to seek and act on opportunities for enhancing patient outcomes.

Coach, mentor and advocate for the Care Management Staff and be the first line, support for Care Management staff questions and/or guidance.

Essential Duties and Responsibilities

 1. In collaboration with the Utilization Manager, responsible to assist with hiring, firing, training and ongoing supervision of Care Management Team, assuring that all staff work by the principles of patient-centeredness and mutual respect with colleague

2. Participates in the development and supervision of department productivity targets and resource management.

3. Oversees Care Management day to day care delivery, ensuring compliance with established regulations and standards, including quality assurance, confidentiality and medical record documentation.

4.   Identifies opportunities and plans strategies to reduce patient length of stay and resource consumption within the Care Management population.

5. Provides direct supervision to all of the Care Management Team, ensuring appropriate and uniform assessments for admission to programs and appropriate care delivery.  Serves as a key problem solving resource to staff on all issues.  Studies patients and Care Navigation Team needs/trends and suggests solutions for process improvement.

6. Develops and maintains tools for auditing and reporting all aspects of Care Management Program, including , but not limited to, capturing appropriate patients, entrance and exit criteria, census per Care Management Team member, including acuities, clinical outcomes, specific disease monitoring and helps to develop improved patient monitoring within their home.

7. Communicates routinely with Hospitals and SNF’s regarding our patient LOS, diagnosis, plan/goal for discharge date, plan for place of disposition, and possible needs at discharge.

8. Provides ongoing education to Care Centers and clinical teams regarding Care Management Program.

9.  Uses customer service principles and techniques to deal with patients and colleagues calmly and pleasantly.

10. Ensures the performance and productivity of all Care Management Staff are evaluated regularly throughout the year and annually.  Collaborates with the Utilization Manager to determine and advise on appropriate corrective actions and develop opportunities for improvement.

11. Maintains a high quality of care delivery by Care Management staff through continuous improvement of procedures, protocols, and standards.  Ensures all Care Management staff is trained in all aspects of the Program.

12. Ensures effective communication and collaboration with multidisciplinary patient care teams.

13. Acts as an intermediary in a collaborative fashion with outside clinical staff from other healthcare institutions, whatever their function.

14. Emphasizes continuity of care and seamless integration during transitions of care to avoid duplication or gaps in plan of care.

15. Networks/communicates with peers to facilitate operation standards and improvements.

16. Performs on-site visits to extended care facilities as needed to facilitate appropriate disposition and ensure elimination of potential gaps in care during transition of care.

17. Models accountability and expects the same from staff.

18. Identify opportunities for health promotion and illness prevention with our patients.

19. Maintains a comprehensive working knowledge of community resources, payer requirements and network services and updates community and clinical team resource tools related to the Care Management Program.

20. Other duties as assigned.
